Collective Society Lifts Form

This form lets us know that your young person needs a lift to and/or from youth this Friday night. This form needs to be filled out every week that your young person needs a lift before Friday 5pm.

Lifts home are given at the kindness of our volunteer youth leaders to young people who have no other way of getting to or from youth. If you or someone else can take your child to and/or from youth; please ask them first so people who have no other way of getting there, do not miss out.

Once the form is filled out, the mobile number listed will receive confirmation on Friday between 5pm and 6pm to confirm that your lift has been organised.

Pick up time is anywhere between 6pm to 7:15pm, and drop off time is between 9:45pm and 11pm. Times are subject to change depending on locations and the quantity of lifts we have each week.

If something changes and your young person no longer needs a lift, please message one of our leaders and we can take you off the list.
